Mine doesn't cover donor fees ($545 per cycle) or specimen storage ($35 per cycle), or what my doctor bills as AI (his actual fee- they cover catheterization, the IUI itself, etc). They do cover everything else. I have a $20 copay for dr. visits and $5 per month for the Clomid.

It sucks royal ASS that some insurance doesn't cover infertility. Ridiculous. Click here to enlarge

I have to pay for the Clomid out of pocket and if/when we start doing IUI again, I think they cover 4 attempts. Everything else we have to pay. Our last insurance was the same way. They covered everything up to the "infertility" diagnosis, all the testing and such. After that, we were on our own. It does suck! Luckily, last time we got pg with DS on our second IUI attempt.

My book says that they will cover everythign to diagnose infertility, but nothing to treat it. They did cover my progesterone and all the testing I had after my miscarriages. And they cover all maternity costs.
