We got in to see the GI doctor

The gastroenterologist had a cancellation, so we were able to get in to see him early. He said that Ian has about the worst reflux he's seen, but the good news is that he is still hydrated and gaining. So, long story short, unless he has failure to thrive issues, they don't like to prescribe anything other than what we're doing already. He has been aspirating it, so we have to keep him on this pillow contraption for 45 minutes after he eats. He also has to be propped on a wedge while he's eating. It looks really weird, but seems to help some. He said we have to keep an eye that he doesn't end up with lung or esophagus problems due to it, but right now he's ok in that regard. He upped his Zantac dose and sent us away with one of these pillows.
